Birth Preferences Sheet
This document is a customizable Birth Preference Sheet (often called a birth plan). It is structured as a checklist and write-in form to help parents communicate their wishes regarding induction methods, pain relief options (both pharmacological and non-pharmacological), birthing positions, delivery of the baby and placenta, and immediate newborn care. The sheet emphasizes the right to informed choice during the medical process.
